Hey there --

I feel so guilty.
My Betta doesn't look good. And I don't have many of these answers, but here goes...

Housing:
How many gallons is your tank? 3/4 gallon
Does it have a filter? No
Does it have a heater? No
What temperature is your tank? 72? I don't know... I fill it a little warmer than room temp.
Does your tank have an air stone or other type of aeration? No
Does your Betta have tank mates? What kind? No

Food:
What food brand do you use? Top fin pellets. Omega One blood worms.
Do you feed flakes or pellets? Pellets, sometimes blood worms
Freeze-dried? Pellets no. Blood worms yes (but they say no risk of parasites on the packaging)
How often do you feed your Betta? How much? Usually twice a day. It used to be about 8 pellets a day, but it's been progressively less (since he isn't responding as well)

Maintenance:
Before your Betta became ill how often did you perform a water change? 10 days
What percentage of water did you change? 100%
Do you vacuum the substrate or just dip out water? Pour all of it out, clean rocks and tree
What additives do you use? None previously, but I recently got API BettaFix

Symptoms and Treatment:
When did you first notice the symptoms? 3 weeks
How has your Betta’s appearance changed? White growth by eye
How has your Betta’s behavior changed? Hangs around at the top of the tree
Is your Betta still eating? 2 pellets a day
Have you started treating your Betta? If so, how? Less food, adding API BettaFix
Does your Betta have any history of being ill? No
How long have you owned your Betta? Was he or she ill or suffering some sort of damage when purchased? 6 months, no

You need to be doing 25% minimum daily water changes.

What temperature is your tank? 72? I don't know... I fill it a little warmer than room temp.
You need a heater the water temp should be 80f

What percentage of water did you change? 100%
100% water changes are stressful to a fish.

What additives do you use? None previously, but I recently got API BettaFix
Bettafix is absolute rubbish the only thing it fixes is API's bank ballance.

You need a good quality water conditioner like Seachem Prime.

+1 to everything NickAu said..your Betta is suffering from ammonia poisoning because of poor water quality and he is lethargic because his water is much too cold. Bettas thrive in 78-82 degrees and in colder temperatures their metabolism slows and their immune system does not function as greatly leaving them more prone to infection and illness. You have a chance at saving him if you buy AT LEAST a one gallon (bigger is better), a heater, and water conditioner to remove harmful chemicals in tap water. I and most people on here suggest Prime.

Bigger is generally easier to take care of, too. 2.5 gallons will fit in quite a small space, and will leave you room for a heater, filter, plants and/or ornaments.

Larger tank, and heater, and water conditioner are the most critical right now.

Also, I would not recommend pouring in water that is warmer than that in the tank -- get it as close to tank temp as possible, otherwise, the fish will experience too much in temperature fluctuations as the water cools down again.
