Any advice?

My betta presents with a superficial, white-waxy pattern along his left side starting around his eye extending laterally and above the gill.

Here are the details and a few pictures are attached.

I will add better pics if I can get them - he's quick and I am a bad photographer.


Housing

What size is your tank? 6 gallon Fluval Edge
What temperature is your tank? 77 F (there is a heater and thermometer)
Does your tank have a filter? Fluval Edge - Max 100 gph/378 Lph; Min 33 gph/125 Lph
Does your tank have an air stone or other type of aeration? No - overflow filer does the job
Is your tank heated? Yes - Marina C10 keeps the tank at a steady 77 F
What tank mates does your betta fish live with? None

Food
What type of food do you feed your betta fish? Nutrafin Max
How often do you feed your betta fish? Every 12 hours, as much as he will eat in two minutes.

Maintenance

How often do you perform a water change? 1/3 once per week.
What percentage of the water do you change when you perform a water change? 33%
What type of additives do you add to the water when you perform a water change? Nutrafin Aqua Plus Tap Water Conditioner.

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Tested by the vendor after cycling with two goldfish - levels were "normal"

Symptoms and Treatment
How has your betta fish's appearance changed? White striations around one eye extending laterally in a caudal direction, but not yet beyond the gills.
How has your betta fish's behavior changed? He's lying down more/less active. Perhaps he is lying down more.
When did you start noticing the symptoms? one week ago - beginning of February.
Have you started treating your fish? If so, how? No medication.
Does your fish have any history of being ill? No. He has been in this tank three months.
How old is your fish (approximately)???

The pet store that I frequent gave me some 'medication'. They did not say (or even know?) what the medication was called, but they said to add 1 drop for every 16 fluid ounces (US).

It looks like methylene blue to me - anyone have any info on/experience with this product?

Just an update: I've found out that the medication from the store is Revive (a mix of methalyne blue and malachite green). It's been two days, and he's still alive in his hospital tank. Right now, he's not eating.

Does anyone have experience with a betta bouncing back from something like this? He's my first betta, and I'd hate for him to die after only three months...
