What is wrong with my mermaid betta: please help

How many gallons is your tank?
20 gallons L
Does it have a filter?
Yes 2 sponge filters
Does it have a heater?
Yes one 100W heater
What temperature is your tank?
79 degrees
Does your tank have an air stone or other type of aeration?
Yes there is air being added by each sponge filter
Does your Betta have tank mates? What kind?
Yes, 3 zerbra danios and 2 african dwarf frogs
Food:
What food brand do you use?
Tetra and Aqueon
Do you feed flakes or pellets?
Both depending on day
Freeze-dried?
Sometimes some blood worms freeze dried
How often do you feed your Betta? How much?
Every other day as he eats the frogs frozen blood worms the most. I put one gumdrop of frozen works every other day and usually some flakes for the danios here and there to make sure they get food as well.
Maintenance:
Before your Betta became ill how often did you perform a water change?
About once every two weeks
What percentage of water did you change?

What is the source of your water?
I start with distilled water and add in everything since my tap water is to hard
Do you vacuum the substrate or just dip out water?
Vacuum
What additives do you use other than conditioner?

Tetra water clarifier
tetra easy balance
imagitaruim aquarium salt
coral life liquid calcium (very small amounts to try and add minerals back to the distilled water)
searchem florish tabs
cappa leaves
and the gravel is fluorite clay

Ammonia: 0ppm
Nitrite: 0ppm
Nitrate: 0ppm
pH: 6.6
Hardness (GH): Very soft - 0-15
Alkalinity (KH): on my strip it looks like it’s reading high - 180-300 ppm
Symptoms and Treatment:
When did you first notice the symptoms?
Yesterday I saw his gills looked a bit discolored or sticking out.
How has your Betta’s appearance changed?
His gills seem black and his fins are very clamped.
How has your Betta’s behavior changed?
He’s starting to spend more time on the bottom of the tank
Is your Betta still eating?
Yes
Have you started treating your Betta? If so, how?
I have not as I do not know the right path of treatment. I was thinking about trying betta fix but I am unsure what is wrong.
