Help!! - Skin infection or Columnaris?

So I have a male plakat betta and I think he either has a skin a infection or Columnaris. He has a while/red discoloration patch on the left side of his face and has has torn/stumpy tail. I’m certain it’s not fin rot. His water was fine since I did a test on it. For the past week I’ve placed him in a seperate container with methylene blue and treating him with 1 dose of kanaplex every 48 hours (I already did that), drops of vitachem, and changing 50% of his water every 3-4 days. I put in a bit of aquarium salt after every water change and I give him some stress guard every day. I even fed him food dosed in kanaplex and Seachem focus to ensure it doesn’t spread internally. I’m not sure if he’s improving though.. If anyone could give me tips I would appreciate it. He also darts at times.

Hi, I’m currently housing him in a seperate container as a hospital tank. So I’ll give you some answers for both.

How many gallons is your tank?
5 (regular tank)
2.5-3 (hospital tank)

Does it have a filter?
Yes (regular tank)
No (hospital tank)

Does it have a heater?
Yes (regular tank)
Yes (hospital tank - added recently due to the weather changes)

What temperature is your tank?
83.8, the tank is on the bottom near the floor so it may be a bit colder. Usually ranges from 80 - 83 degrees F

Does your tank have an air stone or other type of aeration?
Sponge filter (regular tank)

Does your Betta have tank mates?
What kind?
No, only anubias, duckweed, and floaters (regular tank)

Food:What food brand do you use?
Fluval bug bites

Do you feed flakes or pellets?
Slowly sinking granules

Freeze-dried?Yes

How often do you feed your Betta? How much?Once a day, a small pinch usually 3-5 granules depending on the size. I fed him food dosed in Kanaplex, Seachem focus, and garlic guard for about a week. I just did another dose of kanaplex and feeding him good dosed in it.

Maintenance:
Before your Betta became ill how often did you perform a water change?
Once a every two weeks, sometimes I may change it half a week earlier depending on my schedule (regular tank)

I change 40-50% every 3-5 days and add just a pinch of aquarium salt. (hospital tank)
What percentage of water did you change?30 - 40%, I avoid not changing most of it to avoid shocking the fish

What is the source of your water?
Filtered water

Do you vacuum the substrate or just dip out water?Vacuum

What additives do you use other than conditioner? What brand of conditioner?
Imagitarium betta conditioner, Vita chem vitamin supplement, Seachem flourish Water

Parameters:What are your water parameters? Please give exact numbers. If tested by pet store please get exact numbers. "Fine" or "Safe" won't help us help you. Important: Test your water before the regular water change; not after one.

** I only tested these from the regular tank before a water change, the water from the hospital tank isn’t accurate since it has methylene blue in it.

Symptoms and Treatment:When did you first notice the symptoms?
Started out as small tears. I thought it was just because he was clumsy (which he is) but I didn’t know it would end up this bad.

How has your Betta’s appearance changed?Fins are torn, some light scratches on body. Stumpy tail. Red / white patch on his face.

How has your Betta’s behavior changed?
Hides a lot, darts often, usually hides for the whole day.

Is your Betta still eating?
Yes

Have you started treating your Betta? If so, how?
I put him in a seperate hospital tank with a heater and places to hide. Kanaplex every 48 hours, aquarium salt (after every water change), methylene blue (1 dose), vita chem (after every water change, stress guard (every day), few drops of garlic guard (every water change). Feeding him food dosed in kanaplex, Seachem focus, and garlic guard.

Does your Betta have any history of being ill?
No

How long have you owned your Betta?
A little over a year

Was he or she ill or suffering some sort of damage when purchased?

Upon purchase, he did seem have hole in head disease previously due to bad water quality in the cup he came in, but the holes weren’t deep.

Yeah, do you have an idea of what he might have? Possibly a bacterial infection or Columnaris? I’m not sure how he got this way.. only thing I can think of is that he got a cut and got an infection. The kanaplex didn’t help much the first round, so I’m going to do another 3 doses. He’s still eating but he doesn’t seem to have as much energy as he used to and he hides and darts a lot :( I wish there’s more I can do for him.
